We work with you, our patients, to perform the required examination, testing, immunizations and accurate documentation of your form I-693. We believe in the quality of our work. We will gladly redo your paperwork at no additional cost if returned for our error.
Form I-693 expires 2 years from the date it is completed and signed.

His approach utilizes healthy lifestyle with the avoidance of processed foods, decreased sugar intake, decrease carbohydrate intake, exercise and medication as needed. Dee joined the staff of GHFM in December 2012. She graduated with a degree in computer science from Xavier University, New Orleans and a Masters degree in Computer, Information and Network security from DePaul University. Dee became the program director in 2014. She is responsible for the daily operations of the program. Under Dee’s leadership, the program received a 3-year accreditation from CARF in June 2015. She is currently overseeing efforts to expand our services in Ottawa, Illinois.
Joshua Maddox became apart of the Great Heights Family Medicine team in 2017 as the Office manager. Joshua attended Robert Morris University (Chicago,IL) earning an Associates Degree in Paralegal Studies, Bachelors of Business Administration, and currently attending Northcentral University earning his MBA in Healthcare Administration. Joshua Maddox specializes in organizational management and brings to the Great Heights team his expertise in the field of business management, and healthcare operations.
